Case Study: Tummy Tuck (Abdominoplasty)

This patient was a 37 year old 5 foot 4 inch female that weighed 135lbs.  She presented to Dr. Bharti with an interest to improve the appearance of her tummy. She was a very healthy full time mother.  She decided to have plastic surgery under the care of Dr. Gaurav Bharti because she was self conscious of her appearance both in and out of clothing.  She had significant extra skin and fat of the lower abdomen.  In addition, she had a large diastasis (gap) of her abdominal muscles.  This caused her to have a significant bulge in her abdomen most noticeable when she was standing.  After multiple preoperative visits she decided to have a full abdominoplasty (tummy tuck).

She was taken to the operating room as an outpatient to undergo this procedure.  This was performed under general anesthesia.  During this procedure Dr. Bharti removed the excess skin and fat from the lower abdomen (pannus).  He also tightened her ab muscles (rectus abdominus muscles) using permanent plication sutures.  This serves the purpose of narrowing the waste and getting rid of her abdominal buldge.  Once he finished these portions of the procedure he then flexed her bed to be able to perform closure of the incisions using all absorbable sutures.  He then made a new opening for her belly button to be brought through.  Following this, liposuction was then performed of her flanks to further improve her contour.  She had one drain that was left in position to remove excess fluid in the immediate healing period.  Her drain was removed at her one week follow up.  She was then admitted overnight to observe her and make sure her pain was controlled.   The patient was then discharged home the following morning.

While recovering she had to wear compression garmets at all times on her abdomen for a period of 6 weeks using an abdominal binder and spanx.  She walked the night of surgery, but avoided any aggressive aerobic activity or heavy weight lifting for 4 weeks.  In addition, for the first 2 weeks she had to walk around in a flexed position and she slept in a beach chair position. 

Our patient is currently doing very well at three months post operative, and she is very happy with her results.  Her results will continue to improve as she resumes an aggressive exercise regimen and follows a healthy diet.  She will continue to follow up with Dr. Bharti to make sure she continues to be happy.

Case Of The Week: Blepharoplasty (Upper Eyelid Lift) & Congenital Eyelid Ptosis Repair

This patient was a 55-year-old 5 foot 3 inch female that weighed 110lbs.  Her primary concern was that she had significant excess overhanging upper eyelid skin (dermatochalasis) that obstructed her vision and gave her a tired appearance.  In addition, she had congenital left upper eyelid ptosis (eyelid droop) where her left eyelid was in a lower position than her right eyelid with the eyes open.  She was otherwise a very healthy and active person and did not have any history of eye surgery or dry eyes.  She was seen on multiple visits to discuss her options and then plan for her surgery.

Dr. Bharti and her decided to perform a bilateral upper eyelid blepharoplasty (eyelid lift) and a left eyelid ptosis repair.  This was performed in Dr. Bharti’s office procedure room under local anesthesia with the patient awake.  This is a very important part of the procedure when performing the very delicate ptosis repair. Dr. Bharti first cleansed the patient with betadine to disinfect the skin.  The entire time Dr. Bharti was talking and checking on his patient to make sure she was comfortable.  He then carefully marked the incision lines to perform the lid lift procedure to remove the excess upper eyelid skin.  Local anesthesia was then injected.  Ten minutes was then allowed for this to take effect.  After this, the patient felt no pain.

Dr. Bharti then carefully removed the excess skin.  After this was done on both sides he then performed the ptosis repair on the left side.  Dr. Bharti did this by performing a levator palpebrae superioris muscle plication (tightening).  This muscle is the muscle that actually opens your eyes.  After the repair Dr. Bharti then had the patient open and close both of her eyes to make sure that they were symmetric.  Once he was sure that her eyes opened exactly the same he then closed the incisions with a suture that would be removed in one week. 

Once the procedure was completed Dr. Bharti immediately started cold compresses to both eyes to keep swelling controlled.  In addition, the patient was given antibiotic ointment to apply to the incision lines and eye drops.  The cold compresses were continued every hour for the first 24 hours.  She was instructed to keep her head elevated.  The patient was sent home with pain medicines but did not require these.  She followed up in one week for suture removal.  Her next postoperative visits were at 1 month and then 3 months.  On her most recent exam she has a greatly improved field of vision, enhanced aesthetic appearance, and symmetry of her eyelid openings.  She now looks more alert and less tired.  She is very happy with her end result.

Case Of The Week: Arm Lift (Brachioplasty)

This patient was a 39-year-old 5 foot 4 inch female that weighed 120lbs.  She had massive weight loss of greater than 100lbs after gastric bypass surgery.  After the massive weight loss she was left with significant excess skin and fat of her upper arms.   She decided to have plastic surgery under the care of board certified plastic surgeon Dr. Gaurav Bharti.   She was very self-conscious of her appearance both in and out of clothing, and this affected her overall confidence.  After multiple preoperative visits she decided to have an arm lift (brachioplasty). 

She was taken to the operating room in an outpatient surgery center to undergo this procedure.  This was performed under general anesthesia.  During this procedure Dr. Bharti removed the excess skin and fat from her upper arms.  At the beginning of the procedure Dr. Bharti injected local anesthesia to help with pain control and prevent blood loss.  He then injected tumescent fluid and then performed liposuction of the upper arms.  He then in a very careful and stepwise fashion removed the excess skin and fatty tissue closing the surgical wounds rapidly.  All of the sutures used were absorbable.  No drains were necessary.  Steri strips were applied to the incisions, and then a compression wraps were placed.  She was then awoken from anesthesia and discharged home later that day.   Dr. Bharti saw her in clinic the following morning.  She had minimal pain or swelling, and no reports of any numbness.
